During the COVID-19 Public Health Emergency (PHE), we temporarily extended our transition fill supply limit to 90 days for Braven Health℠, Horizon NJ TotalCare (HMO D-SNP) and Horizon Medicare Part D members. A transition fill is a temporary supply of a medication when it is not covered on our formulary or approval is needed.

With the PHE ending onMay 11, 2023, we will return to our standard one-month supply limit for these members beginning onMay 12, 2023. Please be sure to obtain prior authorization or approvals within one month for these members when required. There may also be an alternative medicine that will work for your patient. We encourage you to prescribe these alternate medications when you can.
