We Can Help You Apply for SSI Disability Benefits

Horizon NJ Health works with Healthcare Financial Inc. (HFI) to offer members free screenings and application assistance for Supplemental Security Income (SSI) and/or Social Security Disability Insurance (SSDI). Both SSI and SSDI are federal programs run by the Social Security Administration (SSA) that provide financial help to children with disabilities, and blind or disabled adults.

How do I know if I am a candidate to receive HFI’s help in applying for SSI/SSDI?
To qualify for HFI’s services, you must meet the following SSA requirements:

  • Under the age of 65, and blind or disabled
  • United States citizen or national, or a non-citizen resident
  • Have limited income and resources

If you’re a possible candidate to receive SSI/SSDI, HFI may reach out to you for a free screening and application assistance. If you do not want to wait, call Member Services or your Care Manager to be referred to HFI and get the process started.

How do I start an application for SSI/SSDI?
If you think you may be eligible to receive SSI/SSDI and would like to work with HFI, talk to your Care Manager or Member Services. Whether it’s your first time applying or you are in the middle of the application process and have no outside assistance, ask to be referred to HFI.
